    The Patient Care Coordinator provides a professional, patient-centred reception and administrative service within the HIV and Sexual Health Clinic at 10 Hammersmith Broadway. Key duties include registering patients, managing appointments and clinic attendance, maintaining accurate electronic patient records (EPR), updating demographic information, handling telephone enquiries and supporting day-to-day clinic administration. Working closely with doctors, nurses, health advisers and administrative colleagues, the post holder supports the smooth running of outpatient services by coordinating appointments, assisting with patient enquiries and arranging additional services such as interpreters when required. Check with the patient at every visit that all the above information is accurate • To accurately attend all GUM patients as a new, follow up, reschedule or rebook based on their last visit. • To accurately process all new HIV patients onto Cerner under a medical number. • To effectively diffuse difficult situations with patients and refer to the Administration Manager or Clinic Manager depending on location. • To arrange and book any additional services that may be required by patients attending the clinic e.g. transport or interpreting services. • To maintain patient confidentiality at all times, keeping patients’ information private and only allowing access to it by authorised staff. • Responsible for ensuring that all clinics are encountered, and RTT outcomes are recorded. • To utilise and manage the patient appointment reminder system. • To take part in audits of outpatient activity as required. • To work a flexible Rota system for the Service, including late clinics and weekends where necessary and providing clerical support to other directorate services where needed • Undertake other administration and reception duties at the discretion of the Clinic Manager. • Contribute to Service development of the HIV/GUM Directorate e.g. being involved in multidisciplinary audit/development groups. We are seeking to expand the frailty team in line with the planned redevelopment of the main hospital site. We are looking for clinicians who have passion for frailty and the provision of therapy within acute medicine and same day emergency care. To be responsible to the Lead Chaplain for the day to day provision and development of pastoral, religious and spiritual care for patients, staff, visitors and to confidentially record their activities as part of the chaplaincy data system. 1.     We are looking for a pro-active, motivated individual who enjoys working in a challenging and busy environment in Plastic Surgery within the Specialist Surgery Directorate. The successful candidate will need to have experience of working in the NHS and knowledge of the 18-week wait Pathway. Applicants should be well organised with excellent communicational skills and able to work well under pressure and be a key team player. The appointed candidate will be responsible for booking new patient appointments by priority and date order, uploading referral for clinical triage, pathway management for 18-week RTT and management of our Outpatient PTL (Patient Tracking List). The post is vital to the delivery of timely, consistent, high quality care for patients. Computer literacy and good customer service skills are a must although full training will be given. Main duties of the job • Ensure accurate and comprehensive patient records and clinical details including outpatient referrals are added onto EPR in a timely manner, according to agreed standards, (the Trusts’ Elective Access Policy and Data Quality). • Book appointments on EPR as per Trust procedures in line with the Trusts Elective Access Policy and ensure all referrals are clinically triaged within 48 hours. • Ensure that patients on ERS (Electronic Referral System) are given appointment dates in accordance with clinical priority and length of wait, and that patients on the ASI list are managed accordingly. • Ensure that the appropriate action is taken following both patient and hospital cancellations, such that the utilisation of elective capacity is maximised. • Ensure that the patients’ 18 week RTT pathway is correct on EPR before undertaking any changes. • Be responsible for managing designated patient tracking lists (PTL) and attend weekly PTL meetings with the management teams. • To ensure all clinician annual leave and changes in outpatient sessions are accurately recorded • Take a lead to resolve clinic cancellation issues by communicating with colleagues and arranging cover or alternative clinics where required. • The post holder is expected to develop an understanding of the Trust Elective Access Policy and related procedures, and to make a positive contribution to the ongoing development of elective administration processes. The healthcare support worker will be expected to undertake the following duties under the supervision of a registered practitioner: Patient Care • Assist all patients with their personal care needs, empowering patients or families/carers and actively encouraging participation. This may involve maintaining hygiene needs for individuals such as helping patients to wash and dress themselves and assisting with toileting needs, maintaining privacy and dignity at all times. • Prepare patients, families/carers and the environment for clinical activities supporting patients and their families/carers and preparing specific equipment as necessary. • Participate in the initial assessment of patients’ needs, under the direct supervision of a registered practitioner. • Observe and report the patient’s condition to a registered practitioner as required. • Monitor closely patients having undergone a clinical activity ensuring their safety and wellbeing and reporting to a registered practitioner as required. • Assess and monitor the patient’s skin integrity and report and document immediately any changes or concerns in their condition. • Assist patients to meet their food and fluid intake giving attention to special diets, supplements and cultural requirements.
