The role will lead on the implementation of the priorities as set out in the Bexley Wellbeing Partnership Neighbourhood Plan and support the development of the three Local Care Networks with their delivery plan – with a particular focus on improving outcomes and reducing health inequalities. This role will be a key member of the Neighbourhood Transformation Team, which will support delivering transformational change for the Bexley Wellbeing Partnership – working with the three Local Care Networks and the four Primary Care Networks. This will ensure that the partnership better joins up health and care support, improve people’s health and wellbeing and close the inequalities gap.
