The REACT team is a multi-disciplinary integrated community service within ESNEFT incorporating OPAT (Outpatient Parenteral Antibiotics Therapy), OOHs (Out of Hours) and Virtual Ward pathways patients. Our team delivers excellent patient focused care to patients in the East Suffolk community. Could you be a valued key member of our team? We will offer an individually tailored induction, training and support for the successful candidate. The role involves travelling throughout East Suffolk delivering acute care in patient’s home as a part of REACT nursing team, both during core hours and out of hours. The post holder will be working in an internal rotation. The successful candidate will be expected to work a mixture of shifts that include the following: 11-11 triage shifts, early (0800-1600), late (1500-2300), twilight (1800-2300) and night shifts (2230-0830) including weekends and bank holidays as the service requires. During out of hours between 18:00-8:00 the Nurse will be accompanied with a Health Care Assistant. Pool cars are accessible for staff to use and it is essential that post holder has valid driving licence. In this role, during the core hours, teams’ main responsibilities include working in partnership with the OPAT (Outpatient Parental Antibiotic Therapy) to administer intravenous antibiotics via PICC line or cannula for patients receiving antibiotics in community. Core hours will also include assessment and delivery of care to patients in crisis to allow admission prevention. During the out hours main responsibilities include routine and emergency response to wound care, catheter care for all types of catheters, diabetic care, End of Life care, Anticoagulant medication, Pressure Area care, Venepuncture and Sub cut Fluids in order to ensure 24 hours Nursing care cover in community. You would be liaising with District nursing service to ensure continuity of care and another community services where needed. The role will also include being the shifts team lead, triaging and co-ordinating case load, scheduling visits, carrying out appraisals and clinical supervision for more junior staff. Part of this role is also to support senior management in daily running of the team and promoting change.     We are pleased to offer 1 post to join our Pancreatic Unit in Hull University Teaching Hospitals NHS Trust as a Senior Clinical Fellow in Pancreatic Surgery. We are looking for a highly motivated and ambitious individual looking to further their career in pancreatic surgery. This post is suited to candidates nearing, or who have completed their general surgery training (or equivalent). There are 2 post available, These post's will last for 12 months. The Upper GI Unit in Castle Hill provides tertiary level care to approximately 1.25 million people across North Yorkshire, East Riding of Yorkshire and North Lincolnshire. Around 60-70 major OG and 50-60 pancreatic resectional cases are performed each year as well as a large volume of complex benign UGI cases. Senior Fellows will work alongside a team of 9 UGI consultants, 4 training registrars, a total of 3 senior fellows, 2 junior registrars/fellows, 2 Core Trainees and 9 FY1s, with a variety of subspecialties including pancreato-biliary cancer, oesphago-gastric cancer or complex benign upper GI surgery. They will have the opportunity to benefit from consultant-led training in theatre, access to endoscopy, outpatient clinics and MDTs. Other opportunities within these posts include contribution to research, undergraduate and postgraduate education and management/leadership roles. As well as contributing to the smooth running of the elective wards alongside the other registrars, fellows also participate in the 1:13 (minimum) general on call rota which provides cover to the acute emergency general surgery service in Hull Royal Infirmary as well as out of hours non-resident cover for the elective site Castle Hill Hospital. This will provide experience to emergency care/theatres and multi-disciplinary working in a Major Trauma Centre. As well as providing emergency care to the people of Hull, we also provide tertiary level OG, pancreatic and colorectal care for the surrounding population.     The main chemotherapy unit plus our teenage/young adult area are based within the Bristol Haematology and Oncology Centre. Wessex Neuro ICU is a 13-bed dedicated Unit. The Unit admits 800-1000 patients yearly comprising 300 severe neurological trauma (brain/spinal injuries, complex polytrauma), 300 acute stroke (subarachnoid/intracerebral haemorrhages, ischaemic), 300 neurosurgical cases (100 emergency/200 elective) and neurological (demyelinating polyneuropathies, autoimmune encephalitis etc), medical and surgical cases. Resident Airway Fellows participate in a 1:9 rota. The post is well supported by Consultant input. Successful applicants will have competence in airway management and experience working in anaesthesia and/or critical care. Full attainment of the anaesthetic “Initial Assessment of Competence” (IAC) is mandatory. Applicants with training backgrounds in anaesthesia, critical care and ACCS are welcome. Most successful applicants join at ST3 or above. Support for study leave will be given. The successful candidate will be responsible for supporting the delivery of safe, effective, and patient-centred care while working collaboratively with colleagues across the organisation. (1) Leadership & Management: Daily management of neurocritically unwell patients on ICU. Attendance at ward rounds including stepping up into a senior leadership role, including leading daily ward rounds and being part of the senior decision-making process. (2) Practical Skills & Airway Competence: Resident Airway Fellows will be expected to have competence in practical skills such as preliminary airway management and transfer of the critical care patient; central venous line insertions; arterial cannulations and be able to do - and teach - these procedures independently. Evidence of competence will be required. Prior attainment of the anaesthetic skills demonstrated in the full “Initial Assessment of Competence (IAC)” certificate – or its equivalent - is mandatory. Competence and familiarity with intra- and inter-hospital transfers is essential. Training in lung ultrasound and bedside echocardiography is available from FICE-accredited practitioners. The main duties for the post holder will be to provide support to the Central Booking Service and Operational Team Leader in order to provide a prompt and professional appointment booking service to parents/carers/service users. The post holders will work as part of the CBS Team to ensure that patient appointments are booked with parents/carers over the telephone and also on the RIO system in line with the Central Booking Service processes. The post holder will also ensure that there is prompt and accurate inputting of the clinical activity data onto the RIO system. To work within a team in the Central Booking Service (CBS) providing a comprehensive, efficient and effective appointment booking service in order to meet the Trust’s patient waiting times targets. The Central Booking Service (CBS) is often the first contact a parent/carer will have with the Children & Families Division and may be regarded as the ‘shop window’ for the Community Paediatric, Inclusion Services, Health Visiting Services and Educational Health and Care (EHC). Responsible for the management of clinics and waiting lists, within Community Paediatrics. Inclusion Services (Speech and Language Therapy, Physiotherapy and Occupational Therapy), Health Visiting & EHC 2. Prepare and track clinical records for all patients booked within the CBS, ensuring that appropriate records such as referral letters are filed/stored appropriately in accordance with the Trust’s Policies and Procedures. Therefore ensuring that the records are available to clinicians prior to the patients’ appointment 3. Responsible for the management and booking of appointments on the Patient Administration System (RiO) system as well as the Choose & Book system 4. Responsible for calling the parents/carers of children who have not attended their appointments in order to ascertain the reason why and rebook a new appointment within the appropriate targeted timescales 5. Responsible for carrying out any clinic changes/cancellations on the RIO system in accordance with the CBS Standard Operating Procedures (SoP). Appointment/Pre-Clinic Activity 1. Booking appointments within the appropriate clinic rules and within appropriate timescales in accordance with the CBS SoP, this will include both making calls to parent/carers as well as receiving calls 2. Supporting the directorate on national targets e.g. 18 week Referral to Treatment (RTT), as well as Choose and Book (CaB 3. Ensure Patients are offered a choice of appointment in line with national guidance, wherever possible (see point 2 above). Establishing if parental carers require interpreting services and liaising with Medical Secretaries to make appropriate arrangements. 4. Liaising closely with and responding to the queries from all stakeholders both internally and externally relating to appointments and activity. This will include clinics utilisation, reasons for non attendance and rescheduling clinics as required. 5. Ensuring all referral letters are date-stamped and registered on the patient administration system RIO, within 24 hours of receipt within the CBS to ensure triage is arranged within the clinic rules. . 6. Responsible for collecting missing demographic details from referrals and contacting the referring practitioners or patients directly to ensure completion of the referral documentation. . 7. Ensure that all clinic slots are fully utilised to avoid wasted capacity, where clinics are underutilised inform the Operational Team Leader or Central Booking Service & Activity Manager for appropriate action 8. Ensure that patients who have either i) requested changes to their appointment or ii) have had their appointment changed by the clinicians, are offered appropriate revised appointment dates over the telephone in accordance with 18-week rules 9. Ensure all appointments made and/or changes made to existing appointments are confirmed by letter and recorded on the Patient Administration System (RiO) 10. Ensure all short-notice rescheduling of appointments are completed on RIO and followed up with a telephone confirmation with the parent/carer. Post-Clinic Activity 1. Ensure accurate data inputting of the Community Paediatricians clinical activity onto the RIO system in line with the CBS SoP and Trust Targets (no later than day 7 of the end of the working month) 2. Liaise with Community Paediatricians, Therapists, Health Visiting Teams and Local Authority regarding any queries with data quality to ensure accurate data is recorde 3. Ensure all activity forms are filed appropriately and within a timely manner and are available for audit purposes 4. Provide general non clinical information to patients and staff, while maintaining patient’s confidentiality General duties 1. Responsible for the supervision of the workload and training staff in the Booking Service. This includes covering the work of junior staff when they are on leave 2. Taking part in the rotational tasks within the CBS, which includes ensuring all telephone lines are appropriately covered by staff on a daily basis 3. Ensure all standard operating procedures and codes of conduct are clearly adhered to and updated appropriately in line with guidance from the Central Booking Service &Activity Manager/Operational Team Leader 4. To be involved in, and to undertake, audits and validation/actioning of reports as appropriate, also reviewing work carried out by junior staff 5. Ensure that high levels of confidentiality, in accordance with the Data Protection Act and Caldecott Principles, are adhered to at all times 6.
