As part of the Active Care Group (ACG) Case Management Division, Brownbill Case Management Services (BAL) provides a specialist case management service primarily for individuals who have been affected by brain injury, spinal cord injury, Cerebral Palsy, complex orthopaedic injuries and amputation. We work across the Midlands, London and Home Counties, and South East regions. In this role, you will work closely with the Clinical Lead to provide guidance and support to Case Managers, promoting best practice and service consistency. Additionally, you will play a key role in developing and maintaining strong relationships with existing and prospective referrers, contributing to the ongoing growth and success of the business. You will be responsible for managing your own caseload while providing support to the Clinical Lead and wider Senior Leadership Team to ensure the delivery of high-quality Case Management services across the business. This role is hybrid working, head office Thame and case manager coverage London and Home Counties, Essex, Kent, Midlands and South. Key responsibilities: Manage your own case load of clients with complex disabilities, conducting assessments and setting goals in collaboration with clients and families. Develop and maintain Case Management plans with costed recommendations. Knowledge, Skills and Qualifications: A recognised professional qualification in a healthcare fieldsuch as Nursing, Occupational Therapy, Physiotherapy, or Social Workwith active professional registration maintained throughout employment. Strong organisational skills, including the ability to multitask and organise others. Excellent oral and written communication skills. Capable of working under pressure and adapting to team dynamics.
