This role presents as an excellent opportunity to work in a topical area of service delivery within the NHS right now and will provide the opportunity to gain service development skills. As this is a community-based role, with travel between patient homes, there is a requirement to hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010). You will be responsible for operational delivery of the service, leading and managing a multidisciplinary team. Working closely with a programme manager, you will test new models of care, identifying interventions which make the most difference to this patient cohort, and evaluating the success of the service. Clinically, you will be from a nursing, physiotherapy, or occupational therapy background and will have appropriate clinical experience to be able to support a cohort of residents with moderate to severe frailty who have had 2 or more unplanned admissions within the last year. • To act as an expert clinical resource to the neighbourhood, working clinically as required. • To co-ordinate and maximise capacity within the Proactive Anticipatory Care Service, taking accountability for allocating patient caseloads to clinicians according to need and competence. Allocation should match available resource and the Clinical Operational Lead will be responsible for predicting required resources and ensuring monthly off duties are developed to reflect this demand. • To analyse and interpret patient, staff, and service needs. • To develop and foster relationships and collaborative working with other health and social care agencies and providers. • Ensure referrals into the service are responded to in an appropriate and timely fashion. • To manage relationships and communications with GP Practices within that neighbourhood and escalate concerns as appropriate.
