Main Duties and Responsibilities. this is not exhaustive  Work as part of the multi-disciplinary Mental Health Liaison team to deliver an effective and efficient response to referrals from the acute trust, providing clinical assessments and interventions focusing on relapse prevention, early discharge and improved quality of life for patients referred.  Be expected to undertake mental health assessments for people attending the Emergency Department, offering highly developed clinical and risk assessments and managements plans  Provide a service to the wards within the general hospital, assessing individuals with a range of complex physical and mental health needs. This would include both functional and organic illnesses of later life in addition to younger people  Provide support and training to acute hospital staff regarding recognising mental health conditions and appropriate management and onward referral pathways  Provide information and clinical support to police and ambulance staff in order to help them make appropriate pathway decisions  Provide support and care for individuals brought to an identified Place of Safety under Section 136 of the Mental Health Act and co-ordinate the assessment process. The post holder will need: 1. To work generically in provision of care, to patients under the supervision of a qualified practitioner, following a prescribed treatment/care plan in order to prevent admission to hospital. 2. Plan and prioritise own visits to delegated patients. 3. Ensure patient held records are completed for each visit, and that SystmOne inputting is completed on a daily basis. 4. Make basic judgements on patients’ response to prescribed treatment/care plans, and report findings to a qualified practitioner. 5. Liaise with other health care professionals as required. 6. Communicate with patients/carers by exchanging factual information, reassurance, tact and empathy. 7. Establish and maintain appropriate working relationships with colleagues, patients, carers and other health care professionals. 8. Demonstrate dexterity and coordination when undertaking treatment of patients where accuracy is important, e.g. The taking of bloods, preforming ECGs, taking blood glucose results, wound care, administration of medication such as insulin. 9. Be responsible for ensuring equipment is used safely, following relevant training, and that it is maintained appropriately. 10. Demonstrate an awareness and understanding of consent, and gain consent as appropriate following organisational policies. 11. Share responsibility of indirect patient contact tasks, such as, answering telephones, arranging appointments, processing referrals and inputting activity data. 12. Undertake health promotion / education as required, e.g. smoking cessation, falls etc. 13. May be exposed to bodily fluids, infected material and blood products, therefore must utilise universal precautions and adhere to infection control policies. 14. Attend and contribute to multi-disciplinary team meetings and other meetings where appropriate. 15. Attend in-house and competency training as required in development of current job role. 16. Work predominately independently on specified tasks, with regular clinical support and supervision by a qualified practitioner. 17. Be aware of limitations of own scope of practice and competency, and when to involve a more senior practitioner, both in daily practice and in emergency situations. 18. Acknowledge and recognise peoples’ expressed beliefs, preferences and choices. 19. Demonstrate an awareness of clinical governance and risk management, and apply to work situation. 20. To provide peer support to other assistants. 21. Actively contribute to team working and express ideas on improving services for users and the team. 22. Share responsibility for maintaining store cupboards, cleaning specialist equipment and general housekeeping tasks.     An opportunity has arisen for an enthusiastic and motivated individual to join the Point of Care Testing (POCT) team based at Blackpool Teaching Hospitals NHS Foundation Trust. Although this post is primarily based at Blackpool Victoria Hospital the successful candidate may be expected to carry out work at other sites when required. This is an exciting opportunity to take on a role that has been developed as part of the ongoing Trust POCT improvement initiative. We are looking for an individual on a full time basis who has experience working within a Pathology Laboratory or working with POCT devices and methods in a Healthcare setting. Suitable candidates will require a relevant professional/scientific qualification to degree level or higher, current registration with HCPC and aPost Graduate Diploma (IBMS Specialist portfolio or equivalent). The intended working hours for this position are 8:30am-4:30pm. The post holder will work as part of the Departmental and Multidisciplinary Teams maintaining good lines of communication and promoting a strong commitment to the department. The principal duty of the post holder will be to provide first line support to operators and departments that use point of care testing (POCT) equipment in order to facilitate safe POCT practice throughout the Trust whilst monitoring and facilitating the correct functioning of all POCT devices. The post holder will also oversee the activity of Support Workers and Associate Practitioners within the POCT Team. Additional duties will be undertaken as directed by the Point of Care Testing Lead in consultation with the post holder.
