This dynamic and innovative service provides acute level treatment to patients in the comfort of their own home; Enabling them to be surrounded by the people, routines and comforts which allows them to recover quickly. The service was established in 2013 and has grown rapidly in recent years driven by innovation, technology and a passion for care and treatment outside of traditional ward settings. The service provides a ‘best of both worlds’ whereby patients can receive hospital level diagnostics, specialist input and flexibility to changing medical circumstances whilst patients are in the comfort of their own home. The role will facilitate effective, collaborative nurse-patient relationships which empower patients and their families to be at the centre of acute treatment. The successful applicant will work alongside a diverse range of hospital specialties and professionals from cardiology, palliative care, respiratory, frailty, orthopaedic, surgical, acute medical, emergency teams and more. Typical treatments for patients include acute monitoring of vital signs, symptom management and assessment, venepuncture, acute wound management and the intravenous administration of medication such as diuretics, antibiotics and fluids – all in the patient's own home. Please note that the ability to drive a car is essential for this role. To be a key member of the Acute Hospital @ Home team in ensuring the delivery and maintenance of a high standard of nursing care that is patient centred and evidence based to patients in their usual residence. A min of 2 years clinical practice with experience in the following settings: General Medicine & Surgery, Orthopaedics, Elderly Care, A/E, Critical Care, Respiratory.
