    The post holder will be required to work as part of the Clinical Programme Team with joint working with the wider Digital Team. This role requires an indepth understanding of the EPR (Cerner) applications and other clinical systems (specifically DictateIT) supported by ICT to enable the development of these systems to support the Trusts’ Digital strategy.     Care Home Pathway Coordinator The Care Home Pathway Coordinator will be working within the At Home Team closely with the Transfer of Care Hub (TOCH) and will coordinate all aspects of supported discharges and the single point of discharge from St Helier Hospital to care homes. The service is to ensure care home residents are not unnecessarily admitted to, or delayed in leaving, hospital to the detriment of their wellbeing and independence. The post holder will seek to improve patient flow, reduce delayed transfers of care (DTOC) and avoid unnecessary readmissions, LOS reduction and reduce 4-hour breach for care home residents. We are looking for an enthusiastic individual to take up the role of Care Home Pathway Coordinator. The post holder will be based at the St Helier Hospital and will coordinate the activities related to the Red Bag journey for patients coming into the Hospital from care homes and ensure the Red Bags are received in the hospital and returned to the care homes on discharge of patients after hospital episode. The post holder will work closely with all stakeholders to enable the successful delivery of the Hospital Transfer pathway and the Discharge Process for care home residents. The post holder will facilitate the use of the Transfer Pathway and Discharge activities to support safe and quality care to patients admitted from and to care homes. Ensuring that the hospital teams have relevant information to assess and provide the appropriate treatment plan and that on discharge, care homes have the relevant information to enable continuity of care. 4. Escalate most complex and challenging cases to Senior Discharge Co-ordinators for support or advice. 5. To ensure smooth and timely communication between the Trust and partner agencies involved in patient discharge/transfer of care to avoid unnecessary delays. 8. Ensure the Red Bag Checklist is reviewed, completed, and actioned by the designated responsible staff member when the patient is admitted and discharged. · Ensure the Red Bag remains with the resident while in the hospital and return with a Red Bag containing all the relevant documentation, medication (if required) and personal items. · Track usage of Red Bags and eRedBags by care homes. · Support the process of the Red Bag Scheme in the hospital and SWL ICB including the eRedBag process.     This role focuses on tracking and managing the referral and treatment process for cancer patients, ensuring that those referred under the Suspected Cancer Pathway are diagnosed promptly and that newly diagnosed patients begin treatment within national timescales- Cancer Guidelines V12.1. The individual will be responsible for maintaining accurate tracking systems and producing weekly reports to monitor patient progress. They will also liaise with MDT Navigators across SWL network to ensure that patients requiring treatment are flagged and treated accordingly. In addition to referral and treatment tracking, the MDTC are expected to be involved in capturing and updating patient information in Trust databases, ensuring all relevant details are recorded and up-to-date. The role requires collaboration with the Senior MDT co-ordinators to develop and monitor national and local datasets, including tracking waiting times for newly diagnosed cancer patients. Timely reporting on these metrics is essential to ensure all patients are receiving care in a timely manner This post is for Monday-Friday between 08:00am and 17:00pm on a rota basis. Flexible working cannot be considered for this role. Operational Responsibilities/Support to Departments 1. Assist with the regular review of Multidisciplinary Team Operational Policies and Procedures. 2. Supporting the Cancer Services Management team in the delivery of Cancer National Waiting times and Breaches workshops. 3. To support Cancer Services Management Team in delivering Cancer audits, COSD, staging target CQUIN for the year. 4. Responsible for investigating Breaches of the Cancer Waiting times and for providing the feedback to involved parties. 5. Responsible for producing the resulting Breach Reports, which are submitted to the CSU (Commissioning Support Unit)/CCG(Clinical Commissioning Group) in accordance with reporting procedures. Data Co-ordination and Tracking Cancer Patients 1. To track all Two-Week Rule referrals to ensure that suspected cancer patients receive prompt diagnosis, and to work with other Trust staff to bring forward appointments if necessary. 2. To update tracking systems daily to indicate whether Two Week Rule patients have a benign or malignant diagnosis. 3. To work with Clinicians, Clinical Nurse Specialists, schedulers, diagnostic staff and other cancer data co-ordinators to ensure identification and recording of the Trust’s newly diagnosed cancer patients. 4. The SEN Teaching Assistant opportunities are available on a long-term basis, with both full-time and part-time working patterns available. The successful SEN Teaching Assistant will work across KS1 to KS4, supporting pupils with additional needs in the classroom, assisting with learning activities and helping pupils engage positively with their education.
