Summary
Main Duties of the Job The post holder will be responsible for delivering a range of nursing services within their scope of practice, including: Long-Term Condition Management Supporting the management and monitoring of patients with long-term conditions. Undertaking chronic disease reviews, health assessments and care planning. Promoting patient self-management and healthy lifestyle choices. Supporting recall systems and annual review programmes. Monitoring and interpreting clinical results within competency levels. Clinical Nursing Responsibilities Providing evidence-based nursing care in accordance with local and national guidelines. Maintaining accurate, comprehensive and contemporaneous clinical records. Working collaboratively with GPs and the wider multidisciplinary team. Identifying and acting upon safeguarding concerns in line with practice policy. Contributing to quality improvement activities and preventative healthcare programmes. Treatment Room Services The role may include: Phlebotomy. Vaccinations and immunisations. Cervical cytology (where appropriately trained). Wound care and dressing management. ECGs. Health promotion and screening programmes. NHS health checks and preventative care initiatives. Infection prevention and control procedures.
