Summary

Escalate cases to Band 6/7 staff Follow up on requested improvements by care providers and escalate any unresolved concerns to the Continuing Healthcare Team, Safeguarding Lead, or Quality Assurance Manager Serve as a patient advocate, delivering sensitive information and supporting clients and their representatives regarding FNC/CHC processes Managing a caseload of patients Supporting investigations into an individuals eligibility for NHS Continuing Healthcare following an individual challenge or dispute
