Summary

Support and facilitate the improvement of care and experience for patients with heart failure in the community and hospital. Provide evidence based individualised care for heart failure patients and their carers. To work autonomously assessing individual patient needs, and utilising advanced skills and knowledge to initiate and titrate heart failure medication. Support Heart Failure team with case load management and triaging referrals and provide expert advice on chronic heart failure issues to a wide range of health care professionals and others. Following Trust heart failure therapy protocols, implement and review treatment management plans and respond where necessary to a change in patient condition, refer to other related professionals. To assist in the running of the heart failure support group. Respond to enquiries from relatives and carers Act as patients advocate Demonstrate a caring, nurturing attitude at all times.
