In this role you will actively support the frailty pathways within an evolving integrated service to deliver evidence-based frailty care and admission avoidance pathways across the local health economy.
The successful applicant will be responsible for delivering and supporting high-quality care and acting as an expert practitioner and a role model for the team. You will demonstrate advanced clinical practice skills and decision making, offering a patient centred approach to care planning and maximising independence and well-being of those living with frailty.
