Summary

Personal Care: Help patients with washing, bathing, dressing, grooming, oral hygiene, and using the toilet or commode. Nutrition & Hydration: Serve meals, help feed patients who need assistance, and monitor fluid intake. Mobility & Safety: Help patients move around safely, use mobility aids, or use equipment like hoists. Vital Signs & Observations: Monitor and record temperatures, pulse, respiration, weight, and blood pressure as instructed. Clinical Support (Varies by Setting): Assist with basic clinical tasks such as changing bed linens, restocking supplies, collecting lab samples (e.g., urine or stool), or preparing treatment rooms. Communication & Record Keeping: Update patient notes, report changes in a patients condition to nurses, and reassure families.
