Leeds Children's Hospital has state of the art treatments, facilities and a comprehensive development programme for Nurses What you will bring to the role The CNS outreach team provide follow up support, training and outreach to community based children on LTV and their support systems; continuing care teams / schools / hospices; including planning for transition to adult services. Communication with families can involve such things as helping organise planned admissions for procedures, troubleshooting any issues that arise with equipment or care requirements, or how to go on holiday, home or abroad. Strategically the CNS team feed into the Yorkshire and Humber LTV ODN to ensure the provision of quality evidence based practice and support satellite services regionally to ensure care closer to home is provided whenever possible.
