Summary
Job Purpose To be responsible for providing comprehensive medical care to patients of all ages, ensuring their well-being through accurate diagnosis, effective treatment plans, and ongoing patient education. This role requires a strong commitment to patient care and a thorough understanding of various medical practices. Key Responsibilities Undertaking a variety of duties including surgery consultations, telephone consultations, queries. Administrative duties such as checking and signing repeat prescriptions and dealing with queries, paperwork, reports and correspondence in a timely fashion. The admin requirements will be generated only from your session or a response or results from something you requested. Each session will have a maximum of 13 appointments (15 minutes for face-to-face appointments and 10 minutes for telephone appointments.) Making professional, autonomous decisions in relation to presenting medical problems, whether self-referred or referred from other healthcare workers within the PCNs. Assessing the healthcare needs of patients with undifferentiated and undiagnosed problems. Screening patients for disease risk factors and early signs of illness. Developing care plans for health in consultation with patients and in line with current practice disease management protocols. Providing health education. Recording clear consultation notes to agreed standards. Collecting data for audit purposes and complying with QOF requirements.
