Summary
As a Care Navigator, you will be responsible for receiving, reviewing and processing patient requests, ensuring information is accurately recorded and directed to the appropriate clinical or administrative pathway. You will help patients access the right care by signposting them to appropriate practice, community, external or self-referral services. This is an ideal opportunity for someone who is organised, confident using digital systems and enjoys working with people in a busy healthcare environment. As a Care Navigator, you will help ensure patients receive an efficient, supportive and seamless experience when accessing our services. We're looking for someone with excellent communication and organisational skills who can remain accurate and professional while working in a busy environment.
