About You:

Community Nurse (Band 5) looking for a new and exciting role where you can work your way?

Shropshire Community and provide exceptional care to patients in the Shrewsbury area. This is your chance to make a real difference in

Key Responsibilities:
Deliver high-quality nursing care in the community, empowering patients to maintain independence.
Assess, plan, implement, and evaluate person-centred care plans.
Collaborate with multidisciplinary teams, GP surgeries, and acute trusts.
Assist in facilitating smooth hospital discharges and reducing readmissions.
Clinical skill set required:
- Complex Wound care including Doppler compression
- Simple wound care
- End of life care
- Medication administration
- Catheterisation male female suprapubic
- Bowel care
- Syringe driver
- Insulin administration
- Doppler assessment
- Compression bandaging
Essential Qualifications:

- A minimum of
NMC registration (valid and active).
6 months of NHS experience as a
Community Nurse
.
