Promote a population health approach by identifying unmet need and supporting improved access for underserved communities. Recognise and respond appropriately to safeguarding concerns and escalating levels of risk. Support people presenting in crisis by facilitating access to urgent and specialist services where appropriate. Maintain accurate and timely clinical documentation and records in accordance with Trust policies and professional standards. Participate in service development, quality improvement activities and implementation of neighbourhood health priorities. Provide support, advice and clinical input to social prescribers and other partner organisations where appropriate. Contribute to reducing unnecessary referrals, avoidable admissions and reliance on acute services through proactive intervention and integrated working. Communication The post holder will: Build and maintain effective working relationships across health, social care and VCSE organisations. Communicate effectively with service users, carers, families and professionals. Contribute to MDT meetings, case discussions and service development forums. Share information appropriately to ensure safe, coordinated and effective care. Working Environment The role is community based and requires regular travel across Coventry and Warwickshire. The post holder will work with individuals experiencing a wide range of mental health needs and may be exposed to emotionally challenging situations. The ability to work autonomously whilst maintaining close multidisciplinary relationships is essential.
