Key Responsibilities Undertake structured mental health reviews for patients on the practice's SMI register, ensuring appropriate monitoring, documentation and follow-up. Support medication reviews and monitoring in collaboration with GPs and authorised prescribing clinicians, identifying side effects or concerns and escalating appropriately. Undertake relevant physical health monitoring associated with mental health conditions and treatments where trained and competent. Identify and respond appropriately to safeguarding concerns, following relevant legislation, local procedures and practice policies. Provide appropriate support and signposting for patients experiencing substance misuse difficulties, liaising with specialist services where required. Work collaboratively with community mental health teams, psychiatrists, crisis services, social care and other relevant organisations to promote continuity of care. Maintain accurate, timely and confidential clinical records in accordance with professional standards and practice procedures. Contribute to service development, clinical audits and quality improvement initiatives to improve mental health care within the practice. Person Specification Essential requirements: Current NMC registration as a Registered Mental Health Nurse (RMN). Experience working in a relevant mental health setting. Knowledge of mental health assessment, care planning, severe mental illness and risk management. Understanding of safeguarding, confidentiality and professional boundaries. Excellent communication, organisational and interpersonal skills. Ability to work independently within professional competence and collaboratively as part of a multidisciplinary team. A compassionate, patient-centred and non-judgemental approach. Desirable requirements: Experience working in primary care or a GP surgery. Experience undertaking SMI reviews and supporting patients with complex mental health needs. Experience of medication monitoring, physical health checks and liaison with external mental health services. Additional relevant training in safeguarding, risk assessment, substance misuse or mental health care. This role offers an excellent opportunity for a registered mental health nurse to contribute to the development of mental health services within general practice. The successful candidate will help improve patient access, promote recovery and self-management, and support the delivery of safe, effective and coordinated care. The postholder will work within their professional scope of practice, agreed clinical protocols and supervision arrangements, maintaining current NMC registration and complying with the NMC Code and relevant practice policies.
