Summary

Undertake clinical triage of therapy referrals made to the Referral Hub deciding on the correct care pathway for that patient Carry clinical caseload to support community rehab. To work collaboratively within a multidisciplinary team Work in partnership with Referral Hub staff and management to ensure that patients are allocated to the correct team in a timely way Liaison with acute hospitals, local authorities and other stakeholders to obtain information and resolve queries Participate in the ongoing monitoring and evaluation of the therapy triage, ensuring that all required data is collected, undertaking audit as required. To provide an evidence-based assessment, treatment and advisory service to clients with a variety of conditions. Our Agenda for Change employment contracts are subject to a contractual 13 week probationary period.
