Summary

Kent and Medway Mental Health NHS Trust is seeking a dedicated Registered Mental Health Nurse, Occupational Therapist, Social Worker, or qualified CYP-IAPT practitioner to join our Canterbury CYPMHS team as a Multidisciplinary Pathway Worker.

In this role, you will deliver high-quality clinical assessments, formulation, care planning, and evidence-based interventions for children and young people up to age 18 who present with emotional, behavioural, neurodevelopmental, and mental health needs.

You will manage an allocated caseload within structured CYPMHS pathways, combining autonomous clinical practice with multi-agency working alongside education, social care, primary care, and voluntary-sector partners. Supported by regular clinical supervision and a welcoming multidisciplinary team, you will contribute directly to high standards of care, safeguarding, and service improvement.

• Clinical Assessment & Formulation:Undertake comprehensive assessments covering mental state, risk, safeguarding, and psychosocial needs to co-produce robust care plans and risk-management strategies with children, young people, and their families.

• Evidence-Based Interventions:Manage an allocated caseload and deliver child-centred, family-focused interventions aligned with NICE guidelines and CYP-IAPT principles (such as CBT, Systemic Family Practice, psychoeducation, and parent support).

• Outcome Monitoring:Use routine outcome measures to track therapeutic progress, review care plans, and ensure positive outcomes for service users.

• Multi-Agency Collaboration:Partner effectively with local schools, GPs, paediatric teams, social services, and third-sector organisations to deliver integrated support.

• Governance & Service Improvement:Participate actively in MDT meetings, clinical reviews, audits, safeguarding discussions, and service development initiatives.

• Team Support & Clinical Administration:Maintain accurate, timely electronic clinical records while supporting junior colleagues, trainees, or students and engaging in continuous professional development.

We are Kent and Medway Mental Health NHS Trust. We care for the mental health and wellbeing of people across Kent and Medway.

Our teams support children, young people and adults with a wide range of mental health needs. We also provide specialist services, including all-age eating disorder services and neurodevelopmental assessments for children and young people, such as autism and ADHD.

Because we work across the whole county and across hospital and community settings, we can make care more joined up and easier to access for the people we serve.

Our vision is simple: we are here to help communities not just live with mental illness, but live well. It’s why we’re passionate about working with communities to make mental health care better for everyone. Everything we do is guided by our values: caring, inclusive, curious and confident.

Join us if you share our passion for better mental health care and want to be part of a team that’s working well together.

The post holder must hold a full, valid, UK driving licence to use for business purposes (unless you have a disability as defined by the Equality Act 2010).

Experience & Skills:Demonstrable skill in risk assessment, formulation, and delivering evidence-based therapies to children and young people. Strong communication and multi-agency working abilities.
