Summary

See attached documents for full Band 7 JD Be responsible for delivering the specialist clinical pharmacy service to the assigned clinical specialty during the rotation. Deliver specialised knowledge of medicines and nutrition support to provide advice to consultants, medical, nursing and pharmacy staff on all aspects of medicines use for patients. Deliver pharmaceutical care to patients; recommend, substantiate and communicate best therapeutic options for patients and implement changes in prescribing practice to ensure evidence-based medicine is followed as appropriate. If not already qualified, undertake an independent prescriber qualification within an agreed specialty as agreed with clinicians and funding, complying with all requirements of the GPhC in relation to registration as an independent prescriber and the associated professional responsibilities. Identify medicine-related causes for admission and develop treatment plans to optimise care, deprescribing where appropriate. Undertake risk management for the pharmacy service to allocated wards and ensure compliance with medicines legislation, the Trust Medicines Policy, and medicines related national and local guidelines. Undertake clinical audit of medicine use and pharmacy services within the rotational areas as assigned. Ensure pharmaceutical aspects of research and clinical trials comply with relevant legislation and conduct own practice-based audit/research on pharmaceutical aspects of medicines use. Provide pharmaceutical teaching to pharmacy, medical and nursing staff. Introduce measures to ensure drug expenditure is within agreed limits as requested by the Director and Deputy Director of Pharmacy. This will include monitoring the use and funding of new medicines for the Medicines Prescribing Sub-Committee (MPC)
