Summary
Your role will include: Providing face-to-face, telephone and digital consultations, including home visits where required. Managing a broad range of acute and chronic conditions, minor illness and injuries. Providing continuity of care and developing strong, trusted relationships with patients. Prescribing safely and appropriately and requesting and interpreting diagnostic investigations. Providing preventative healthcare, health promotion, screening and vaccination advice. Recognising and appropriately escalating safeguarding concerns. Working with the wider MDT and external healthcare professionals to coordinate care for patients. Supporting delivery of QOF and other contractual and quality requirements. Participating in clinical governance, audits and quality improvement. Keeping up to date with current clinical guidance, evidence-based practice and developments in primary care. Maintaining your CPD, mandatory training and professional development.
