Summary
An exciting opportunity has arisen for a compassionate and motivated Band 6 RMN to join our Older Adult Community Mental Health Team (CMHT) based at Charnley Fold, Bamber Bridge.
Our multidisciplinary team provides specialist assessment, treatment and support to older adults across Central Lancashire experiencing functional and organic mental health difficulties. We work collaboratively with service users, families, carers and partner agencies to promote independence, wellbeing and quality of life.
The service operates Monday to Friday, 9:00am to 5:00pm (excluding bank holidays).
As a Band 6 RMN, you will manage a caseload of older adults with complex mental health needs, undertaking assessments, care planning, risk management and evidence-based interventions. You will work autonomously whilst contributing to multidisciplinary reviews and collaborating with primary care, social care, inpatient services and community partners to deliver coordinated, person-centred care.
We are looking for a Registered Mental Health Nurse with excellent communication, clinical assessment and decision-making skills. Experience within older adult or community mental health services is desirable, although we welcome applications from nurses with a genuine passion for older adult mental health and delivering high-quality care.
In return, you will join a supportive and experienced team committed to professional development, clinical excellence and service improvement.
The Older Adult Community Mental Health Team provides assessment, treatment and ongoing support for older adults experiencing complex mental health difficulties, including severe and enduring mental illness, dementia, depression, anxiety, psychosis and mental health conditions associated with ageing, frailty and physical health comorbidities.
The post holder will work as an autonomous practitioner within a multidisciplinary team to undertake comprehensive mental health assessments, develop and review care plans, coordinate care, manage risk and provide evidence-based interventions. The role requires excellent clinical judgement, effective communication skills and a commitment to person-centred, recovery-focused and strengths-based practice. The post holder will support service users, carers and families to achieve positive outcomes while ensuring care is delivered in accordance with professional standards, legislation and Trust policies.

Our aim is to reflect the communities we serve, so we particularly encourage applications from under-represented groups. If you’d like to discuss your application, please ask.
We also welcome individuals with lived experience relevant to the role or service area, as your insight brings authenticity to our services.
