Please see the job description.Key responsibilities include: Leading and managing the multidisciplinary Community Stroke and Neuro rehabilitation team, and the Falls team for Ealing. Responsibility for the day to day operational management and service delivery Acting as a role model in expert practice facilitating and supporting the delivery of effective patient care. Developing and monitor clinical outcome metrics for designated areas proposing and implementing improvements where they arerequired. Acting as an ambassador for the service developing robust communication strategies that provide all stakeholders with the opportunity to be engaged in service developments and informed on progress. Developing partnership opportunities and integrated working with all health and social care providers ensuring personalised quality carefor patients. Ensuring high standards of clinical practice through education and training to staff and students. Participating in the transformation of services within Ealing Community Rehabilitation.
