Summary
Process repeat and acute prescription requests in line with practice policies and prescribing protocols. Ensure prescriptions are accurately prepared and authorised within agreed timescales. Identify and escalate medication queries, prescribing issues and patient safety concerns to the appropriate clinician. Monitor medication review dates and ensure patients are recalled when reviews are due. Act as a point of contact for patients, pharmacies and healthcare professionals regarding prescription enquiries. Support and provide guidance to members of the prescriptions team. Assist in reviewing and improving prescribing administration processes. Support medicines optimisation initiatives and prescribing audits. Process medication-related correspondence received from hospitals and other healthcare providers in accordance with practice protocols. Maintain accurate patient records using the practice clinical system. Contribute to quality improvement activities and prescribing safety reviews. Comply with all information governance, confidentiality and safeguarding requirements. Provide day-to-day supervision and support to prescription clerks. Assist with training, induction and ongoing development of prescription staff. Monitor workload distribution within the prescriptions team. Support the implementation and review of prescribing procedures. Escalate concerns, incidents or risks to management promptly. Assist in developing standard operating procedures relating to prescribing administration. Work closely with the practice pharmacist, pharmacy technicians and clinicians. Support medicines optimisation initiatives. Identify patients requiring medication reviews and recall activities. Assist with audits relating to prescribing safety, high-risk medications and prescribing performance. Monitor compliance with prescribing protocols and guidelines. Support reduction of prescription waste through patient education and appropriate administration processes.
