Optimum Care Services (Nottm) Ltd is seeking compassionate, patient and reliable Adult Day Services Support Workers to support adults across County Durham in accessing meaningful activities, developing independence and participating in their local communities.

The role involves providing person-centred social care and practical support to adults with a range of assessed care and support needs, including older people, adults with physical disabilities or sensory impairments, learning disabilities, autism, challenging behaviour and mental health social care needs.

Responsibilities may include supporting individuals to participate in social and recreational activities, develop daily living and independent living skills, access community facilities, build confidence, maintain relationships and engage in volunteering or employment opportunities where appropriate. The role may also involve befriending, one-to-one support, outreach activities, rehabilitation and re-enablement, and providing respite support for carers in line with the individual's assessed needs and care plan.

The successful candidates will promote dignity, privacy, choice, independence and social inclusion while ensuring the safety and wellbeing of the adults they support. Support must be flexible, respectful and tailored to each individual's Care Plan, Support Plan or Person-Centred Plan.

We are looking for dependable team members who can build positive relationships with service users, encourage participation and independence, and contribute to a safe, inclusive and supportive environment.
