Summary
We are seeking a Nurse Practitioner who will provide high-quality, patient-centred care to patients presenting with a wide range of acute and ongoing healthcare needs. The role will include: Assessing, diagnosing and managing patients presenting with minor illness and acute conditions, working autonomously within their scope of practice. Providing appropriate clinical advice, treatment and safety-netting to patients, ensuring they receive the right care at the right time. Supporting patients with their ongoing medical and nursing needs, including contributing to the management and review of patients with long-term conditions. Requesting and interpreting appropriate investigations and making referrals to other healthcare professionals and services where clinically indicated. Undertaking home visits as required, providing assessment, diagnosis, treatment and ongoing management to patients who are unable to attend the practice. Working collaboratively with GPs, Nurse Practitioners, Practice Nurses, Healthcare Assistants and the wider multidisciplinary team to ensure continuity and high standards of patient care. Promoting health, wellbeing and self-management, whilst ensuring patients are fully involved in decisions about their care. Maintaining accurate and comprehensive clinical records and working in accordance with practice policies, professional standards and relevant clinical guidelines.
