Summary
Accountability: Reports to the Lead Practice Nurse for day-to-day management, Practice Partners for clinical and professional matters, and the Practice Manager for employment issues. Purpose of the Role: To provide high-quality, patient-centred nursing care, supporting the assessment, treatment and management of patients with acute, chronic and long-term health conditions. The role promotes health and wellbeing, disease prevention and self-management, working collaboratively with the multidisciplinary team to deliver safe, effective and evidence-based care. Key Responsibilities: Assess, plan, deliver and evaluate care for patients with acute, chronic and complex health needs. Undertake health promotion, screening, immunisations, chronic disease monitoring, wound care and medication reviews within scope of practice. Support patients to improve health outcomes Recognise and respond appropriately to urgent clinical situations, referring when required. Communicate effectively with patients, carers and healthcare professionals to ensure coordinated care. Maintain accurate clinical records and practise in accordance with NMC standards, national guidance and practice policies. Contribute to audits, quality improvement, safeguarding, infection prevention and risk management activities. Liaise with community and specialist services to support continuity of care. Maintain professional development and support the learning and development of colleagues and students where appropriate.
