Monitor and optimise diabetes treatment in line with national guidelines. Undertake chronic disease management clinics. Administer vaccines and immunisations. Perform cervical screening, wound care and health promotion activities as appropriate. Maintain accurate clinical records and contribute to quality improvement initiatives. PLEASE NOTE, PREVIOUS EXPERIENCE OF WORKING AS A PRACTICE NURSE AND A QUALIFICATION IN DIABETES ARE ESSENTIAL FOR THIS POSITION.
