Summary

This is an exciting opportunity to join Conwy’s Mental Wellness Team as a Social Worker.

The Mental Wellness Team is a specialist Social Work Mental Health Team that works closely with partner organisations and other services to provide specialist support and interventions for people experiencing complex needs as a result of ongoing psychosocial difficulties.

As a Social Worker within the team, you will undertake assessments and provide therapeutic interventions, alongside consultation, guidance and advice to professionals and other frontline staff. You will also work directly with individuals and their families, offering advice, support and signposting to appropriate services, as well as facilitating onward referrals where required.

The role will have a particular focus on supporting individuals for whom the Local Authority has Section 117 aftercare responsibilities.
