Job description Job title: Advanced Nurse Practitioner (ANP) / Advanced Clinical Practitioner (ACP) Location: Extended Access Hub, Belmont Health Centre, Harrow Employer: Health Alliance PCN Hours: Monday to Friday 16:00 to 20:00, Saturday 09:00 to 14:00. Regular fixed slots, maximum three shifts per week. Pay: Competitive locum hourly rate, dependent on experience and clinical scope. Job summary Health Alliance PCN is looking for an experienced ANP or ACP to deliver clinical care within our Extended Access Hub. The service is mostly a minor illness clinic, with 15-minute appointments, and the post-holder will see patients aged 2 years and over. The role is site-based, with no home visits. You will work autonomously within your scope of practice as an independent prescriber, as part of the wider PCN team. Clinical responsibilities Assess, diagnose, treat and manage patients aged 2 years and over presenting with minor illness and other undifferentiated acute conditions. Review and manage patients with long-term conditions where appropriate within the Extended Access service. Prescribe as an independent prescriber, in line with local formularies, national guidance and safe prescribing standards. Request, interpret and act on investigations where clinically appropriate. Use sound clinical reasoning to identify and manage risk, recognise the acutely unwell patient, and escalate or refer to urgent, emergency or onward care as needed. Provide clear safety-netting advice and appropriate follow-up. Manage your own clinic, working to 15-minute appointments. Professional and team responsibilities Keep accurate, timely and contemporaneous records on the clinical system (EMIS). Communicate clearly and compassionately with patients, carers and families, and with colleagues across the Hub and member practices. Work as a reliable member of the Extended Access team and attend your agreed sessions consistently. Pass on relevant information to the patient's registered practice where follow-up is needed. Governance and development Follow PCN and practice policies, including safeguarding (adults and children), infection prevention and control, information governance, health and safety, and clinical governance. Report and learn from incidents, complaints and significant events. Take part in audit, quality improvement and service development where appropriate. Maintain your professional registration, revalidation, mandatory training and continuing professional development.
