WE Flourish is a PCN-wide, non-emergency health and wellbeing service for adults aged 18 and over. This role has a particular place-based focus on Kirk Hallam and the Sport England Place Partnership, while operating through the wider WE: Flourish Single Point of Access rather than as a separate access route. Job Summary The Health and Wellbeing Coach will provide Tier 2 coaching and supported intervention for adults registered with an Erewash PCN practice, with a particular focus on residents of Kirk Hallam. The post holder will use personalised, strengths-based coaching to support health, wellbeing, confidence, motivation and self-management, and help people access appropriate lifestyle and physical activity opportunities. Please note this role is not a clinical role. General Duties At all times, the post holder will behave professionally, promote high-quality person-centred support, contribute to a positive team culture and foster continual improvement across WE Flourish, EHP and EPCN.
