Summary

Duties and Responsibilities

• Provide timely high quality clinical care and assessments during base visits (20 minute

appointments)

• Undertake home visits quickly and efficiently whilst providing high quality effective care.

• High quality telephone triage calls managing most patients over the phone. The service

average for completing and closing a case without further face to face review sits between

70-80% of calls.

• All GPs regardless of shift type worked, should be working on the telephone triage lists when

there are otherwise no base or home visits to complete.

• When only working on telephone triage or when there are no patients to see in base or at a patient’s home, you should aim to complete on average 4 high quality telephone triage calls per hour.

• When working through the triage list please prioritise call backs to palliative, high priority

and urgent referrals first. You should periodically look at lower priorities and identify and call any patients that might need a more urgent call back or when cases in the top three priorities are low.

• Please do not simply focus on callbacks to cases that might seem quick and easy neglecting referrals where 111 have indicated a more urgent priority

• Antibiotic prescribing should follow the SCAN antibiotic guidelines SCAN Guidelines

• Keep good up to date written medical records that should clearly record information to

answer the criterion defined in the national audit toolkit for urgent care -

• To work with and support other clinical and operational team members with clinical or

operational matters that may arise from time to time during the live hours of the service

• To work in close partnership and liaison with primary care, acute and specialist services,

including but not limited to Urgent Community Response (UCR), Hospital at Home, Single

Point of Access (SPA), Community Services, social services, mental health services and

voluntary services where possible to manage patients safely in the community without the

need for hospital admission.

• To receive and review clinical and operational updates from the medical leadership team.

These are usually provided every week by the medical advisor and will be forwarded to your

employing agency to disseminate to any of their agency GPs that are undertaking sessions of

work in this service.

• To arrive on time ready to start your shift for your booked time and work up until the end of

your session(s) of work.

Further information

• Base visiting locations (clinic appointments) are in Banbury, Bicester, Oxford, Witney,

Abingdon and Henley.

• A typical session is 4-5 hours in length which can be combined with further sessions to

follow on i.e., back to back sessions.

• Overnight sessions are usually 9 hours in length (11pm to 8am) and are usually based out of

Banbury, Oxford, Abingdon and occasionally Witney. The Working Time Directive advises an

11-hour rest period between working days.

• OHFT do not pay for meal breaks (for agency GPs) and will not process invoices (timesheets)

where this has not been indicated. If this is not recorded on the timesheet, the urgent care

admin team will add this to a timesheet and forward for payment of clinically worked hours.

• Agency GPs should take a meal break as follows:

▪ Shift 6 hours or less = No break required

▪ Shift more than 6 hours, but no more than 10 hours Break = 30 minutes

▪ Shift more than 10 hours Break = 1 hour

• During your rest period, all our clinic sites have staff rest rooms where you can rest and take

food. We do not advise sleeping during a rest period.

• If you are taking a break away from the rest room, please inform the receptionist/driver

working with you, where you will be and remain easily contactable and be prepared to act

immediately on any urgent clinical matters that require your clinical attention. You are still

on call for urgent clinical matters during a rest period.

• Sleeping in consultation rooms and on patient couches is unprofessional and is not allowed.

As above we have staff rest rooms where you can take rest.

• If there is no clinical work to be done and you are not on a rest period, you should be logged

on, awake and sat at your desk ready to call patients as they come through on Adastra.

• For home visiting sessions and overnights, the GP will be accompanied by a driver in a trust

vehicle with equipment provided necessary for examining a patient, including key

medications, a defibrillator, Oxygen, and nebulisers.

• A rugged laptop computer is provided for use on home visits. This permits you to access

triage notes and write up and close your case before moving to the next visit.

• Many of our clinicians now wear their own scrubs to work. You may choose to wear other

suitable and professional looking attire. Jeans and sportswear when undertaking patient

facing work is not acceptable and unprofessional.

• We are required to regularly audit the work of our clinical workforce using the audit toolkit

mentioned above. We will provide a feedback report to you called the NQR report. This is

designed to support your ongoing professional learning and development which you should

submit as part of your annual NHS appraisal.

• Where your scores are substandard or require improvement, we will engage and support

you to improve your clinical work and audit scores. Where you are not able or willing to

engage, or your scores remain poor we reserve the right to notify your agency and you may

be barred from undertaking any further work within the trust.
