Summary
Key Responsibilities Clinical Duties Undertake face-to-face, telephone, and video consultations Perform holistic clinical assessments and examinations Diagnose and manage acute presentations within scope of practice Carry out home visits for housebound or vulnerable patients Develop and implement management plans Escalate or refer patients to GPs or secondary care when appropriate Support continuity of care within the practice Urgent Care & Triage Manage same-day urgent appointments Triage patients based on clinical need and risk Support reduction of GP workload through autonomous practice Prescribing & Medicines Management (if qualified) Prescribe safely as an Independent Prescriber (if applicable) Undertake medication reviews Work within national and local prescribing guidelines Multidisciplinary Working Work collaboratively with GPs, nurses, pharmacists, and wider PCN teams Participate in MDT discussions and care planning Contribute to integrated patient care across services Governance & Quality Maintain accurate, timely clinical records Participate in audits, quality improvement, and significant event reviews Comply with safeguarding policies and clinical governance frameworks Professional Development Maintain registration with the relevant professional body Engage in CPD and ongoing training relevant to primary care Participate in appraisal and revalidation
