Summary

The Practice Nurse supports safe, effective patient care across infection control, long-term condition management, and population health. Core duties include working to IPC policy, assessing patient needs, and contributing to quality improvement. The role develops and delivers care plans for frail or housebound patients and supports public health programmes such as immunisations, cervical screening, and opportunistic health promotion. Population health approaches are embedded to reduce inequalities and improve access. The nurse provides evidence based routine care, teaches patients and families about condition management, and supports new clinicians. Responsibilities include participating in audits and research, updating protocols, and working within national and local guidelines. The nurse recognises professional boundaries, supports enhanced services, adheres to safeguarding legislation, and assists with complaints and learning from incidents. They complete mandatory training, contribute to clinical governance, review policies, and attend annual appraisal. Additional duties include leading public health campaigns, offering travel advice, providing wound care, maintaining accurate coded records, supporting QOF targets, managing chronic disease registers, maintaining a safe workspace, chaperoning, assisting in emergencies, and liaising with external agencies for vulnerable patients. Other tasks may be required depending on workload and staffing
