Summary
This is an opportunity for a qualified Social Worker, Occupational Therapist or Nurse
to join our established and experienced multi disciplinary team. You will be working as a Hospital Discharge Coordinator covering the two Older Adult Mental Health teams in Monmouthshire, providing assessment and care planning, including a specialist Mental Health and Care and Treatment Planning service to meet the presenting needs of individuals and their carers.
