Summary

Job description Job title: Practice Nurse Hours: Approximately 20 hours per week Responsible to:Practice Manager Location: The Ridgeway Surgery and Alexandra Avenue branch site Job purpose To work as part of the practice nursing and wider multidisciplinary team to provide safe, effective and patient-centred nursing care to the practice population. The postholder will undertake a broad range of Practice Nurse duties according to their experience, training and assessed competencies. They will work collaboratively with colleagues, make effective use of patient contacts and contribute to the continued development of the practice nursing service. Main responsibilities Practice nursing Provide general Practice Nurse services including, according to competency: Cervical screening and associated health promotion. Childhood immunisations in accordance with the national immunisation programme. Adult immunisations and vaccinations. Seasonal vaccination programmes including influenza and other programmes commissioned by the NHS. Wound assessment, dressings and ongoing wound care. Administration of prescribed injections and medicines in accordance with appropriate authorisation. Long-term condition reviews and monitoring within areas of competence. Diabetic monitoring and foot assessment where appropriate. Other treatment room procedures appropriate to the Practice Nurse role. Health promotion, lifestyle advice and preventative healthcare. Supporting patients to understand and manage their health conditions. The postholder will recognise the limits of their own competence and seek appropriate advice or escalate to another clinician where required. Patient care and team working Provide high-quality, compassionate and individualised care. Work collaboratively with Practice Nurses, HCAs, GPs, advanced practitioners, pharmacists and other members of the multidisciplinary team. Make effective use of each patient contact and, where clinically appropriate and practical, undertake additional relevant nursing interventions to support joined-up care and avoid unnecessary repeat appointments. Support colleagues and contribute flexibly to the day-to-day delivery of the nursing service. Respond appropriately to urgent clinical issues arising during nursing contacts. Communicate effectively with patients, carers and colleagues. Participate in nursing and practice meetings as required. Clinical governance and safety Maintain accurate, contemporaneous clinical records using EMIS Web and other practice systems. Work in accordance with NMC professional standards, practice policies and relevant national guidance. Work within Patient Group Directions (PGDs), Patient Specific Directions and other appropriate medicines authorisations. Follow infection prevention and control procedures and contribute to maintaining high standards within clinical areas. Follow cold-chain and vaccine storage procedures where relevant. Ensure clinical equipment is used safely and report faults or concerns promptly. Participate in clinical audit, significant event review, quality improvement and other governance activity as appropriate. Maintain patient confidentiality and comply with information governance and data protection requirements. Follow safeguarding policies and procedures and raise concerns appropriately. Training and professional development Maintain NMC registration and meet professional requirements for continuing professional development and revalidation. Maintain competencies required for the role and undertake appropriate updates and training. Be willing to develop additional competencies according to the needs of the practice and the postholder's scope of practice. Participate in appraisal and identify development needs with the practice. Support a culture of learning and development within the nursing and HCA team. Service development Contribute positively to the development of the practice nursing service. Support effective use of skill mix across the Practice Nurse and HCA workforce. Participate in new clinical programmes and service improvements where appropriate. Identify opportunities to improve patient care, efficiency and the patient experience and share these with the wider team. Work constructively with the Lead Nurse and management team to respond to changing patient and service needs. General The postholder will be expected to work across the practice's sites where required and to undertake other reasonable duties appropriate to the nature and level of the role. Any significant changes will be discussed with the postholder.
