The post holder will provide and maintain a high standard of Nursing care for patients as well as providing Nursing assistance to the Doctors and other members of the primary care team, working together to deliver care within the boundaries and scope of their role, focusing upon nursing treatments, supporting patients to be healthy, monitoring of long-term conditions, health prevention and screening activities. S/he will work collaboratively with the general practice team to meet the needs of patients, practice key performance indicators and targets, e.g., QOF, support the delivery of policy and procedures and provide nurses leadership as required to healthcare assistants and other workforce colleagues. This contract is permanent, the successful applicant will serve a six months probationary period, with a view to salary review on completion of probation. The Post holder will be expected to provide typical practice nurse duties, such as Chronic Disease Management, Cytology, Childhood imms, etc. Clinical Responsibilities Provide high quality patient care and treatment. Participate in setting standards for care and auditing clinical outcomes. Promote and participate in maintaining a safe comfortable and therapeutic environment. Participate in developing strategies and promoting health promotion programs for patients and carers and facilitate screening clinics. Take responsibility for the safe storage and administration of drugs and equipment. Provide nursing care in a variety of settings by holistic assessment, implementation, and evaluation of programs of care as appropriate. Participate in setting and monitoring standards to ensure effective care ensuring delivery of evidence-based practice. Provide information, advice and support to patients face to face and via the telephone Involvement in team discussions to ensure best practice Key Skills Clinical skills Assistance with Coil Fitting Urinalysis Infection control management of equipment Immunisations / vaccines (Child & Adult) Contraception advice General Family planning advice Cervical Smear testing Health promotion and lifestyle advice Chronic disease management. Leadership: All employees must, Promote best practice Provide pier support and clinical leadership within multi-disciplinary teams Support learning from audit, significant event reviews, and critical incidents. Clinical: All employees must, Deliver medical services via surgeries, clinics and visits as defined via your work schedule. Make professionally autonomous decisions in relation to presenting problems, whether self-referred or referred internally within the organisation. Receive patients with undifferentiated and undiagnosed problems and make assessments of their healthcare needs. Screen patients for disease risk factors and early signs of illness and deliver national and local initiatives. Provide advice and health education and develop with patients, a healthcare plan (if applicable). Referral of patients as appropriate to other services, either electively or as an emergency. Review clinical correspondence about, and results of investigations on, patients. Taking appropriate and timely actions and recording good clinical notes. Be prepared to visit patients in their own homes and when agreed, take ad-hoc duties. Make yourself available to cover annual leave of colleagues, by agreement. Training and Education: Record all relevant training and personal development undertaken in a personal development plan (PDP). Undertake annual appraisals and post appraisal reviews meetings as specified. Ensure sufficient time is spent on personal development and appropriate education (Including statutory and mandatory training) to ensure your skills, knowledge and attributes are refreshed and current. Attend formal reviews of personal and service progress to assist in the further development of the service. Training and Development of clinical computer system (where applicable). Development of screening, primary care and secondary care prevention, and chronic disease management services (where applicable). Clinical Governance / Research and Audit: Provide updated training certificates, qualifications, and appraisal evidence upon request. Contribute to the evaluation and clinical standard setting with colleagues. Work together with the service to deliver the objectives of clinical governance. Communication: Attend and participate in regular service meetings. Development of close links and liaison with other agency and secondary care organisations for the benefit of patients care. Co-operate with Prescot Medical Centre to ensure all statutory and mandatory requirements are complied with, in particular, compliance with the health and social care act 2008, Code of Practice for the prevention and control of healthcare associated infections. Be knowledgeable of and compliant with Prescot Medical Centres infection control policy and procedure, which includes attendance at mandatory training events. Health and Safety: All employees (Including temporary workers) will be expected to take responsibility for their own health and safety, wellbeing and security and assist in the maintaining the health and safety, wellbeing, and security of the working environment for others. Employees (Including temporary workers) must not intentionally misuse or interfere with anything provided in the interests of health and safety, eg: misuse of equipment. It is a legal requirement that all employees (Including temporary workers) attend training provided by the employer and report to their manager without delay, any accidents, incidents, and defects in equipment, place of work or system of work which might endanger safety, health, or security of which s/he becomes aware of.
