Summary
To assist the senior team in providing care and physical rehabilitation to people who require short team period of recovery in their own homes following an acute event or deterioration that may or may not have resulted in an acute hospital stay. The aim is to provide person centred rehabilitation to enable patients to reach their maximum potential and live as
independently as possible.
To work as part of the multidisciplinary team and when necessary on own initiative.
To provide direct care and rehabilitation to the patients served by the team.
To act in a manner that protects and maintains the patient’s sense of dignity and respect, while observing their individual religious and cultural practices.
Encourage patients to participate in activities of daily living and facilitate their active involvement in therapeutic activities.
Assist the patient work towards to rehabilitation goals to enhance independence and wellbeing.
Ensure active involvement in manual handling of patients as required – in line with the Trust’s Manual Handling Policy.
To work within the guidance of the Infection Control Policy and wearing appropriate protective clothing at all times.
Observe record and report changes in the patients’ health and environment to senior staff.
To be familiar with the steps to be taken in medical emergencies and act accordingly.
Complete observations including urinalysis and blood sugar monitoring as required.
Assist the senior team with prelim visits, first visits and review visits to patients.
Work directly with the therapists within the team to support them providing effective assessment and treatment including in the in-patient beds if necessary.
Assist in planning work together with the senior team.
Assist in contributing to care planning and risk management plans.

In accordance with EPUT confidentiality policy. Being aware that any information received whilst on duty if confidential.
Act as direct support to senior members of the team including the Team Leader, associate practitioners, nurses and therapists to ensure the effective service provision.
RESPONSIBILITY FOR PATIENT/CLIENT CARE
Encourage patients to participate in activities of daily living and facilitate their active
involvement in therapeutic activities.
Assist the patient work towards to rehabilitation goals to enhance independence and wellbeing.
Ensure active involvement in manual handling of patients as required – in line with the Trust’s Manual Handling Policy.

Collect patient specimens as required.
Observe and record dietary needs and fluid intake as required.
Assist the senior team with prelim visits, first visits and review visits to patients.
Work directly with the therapists within the team to support them providing effective
assessment and treatment including in the in-patient beds if necessary.
Assist in planning work together with the senior team.
Assist in contributing to care planning and risk management plans.
COMMUNICATION AND WORKING RELATIONSHIPS
Ensure excellent communication skills with patients and carers at all times.
Ensure adherence to record keeping policy and the use of Systm One achieving accurate records and reporting any changes to senior staff with due regard to confidentiality.
Establish and maintain a good professional working relationship with colleagues and other professionals both internal and external to the team.
Be responsible for working with colleagues, seeking to improve the quality of services provided to our patients.
Be responsible for communicating accurate information assisting with office based tasks including the team data base, archiving etc.
Be aware of your working environment and report any hazardous situations to the senior in charge.
Be flexible co-operative and willing in undertaking allocated team tasks.
Assist with induction and teaching of new staff/visitors.
INFORMATION SYSTEMS
To be able to use a range of IT systems as required by the service to support effective working.
To be able to communicate electronically, i.e. using emails and use of trust intranet.
To be able to complete all required electronic records on Systm One.

PERSONAL CONDUCT AND DEVELOPMENT
To ensure all mandatory training requirements are met via a mix of e-learning and face to face training.
Undertake clinical supervision to enhance practice and accept constructive criticism to benefit safe practice.

To be appraised annually and ensure objectives support the on-going development of the team.
To be aware of one’s own self-development need and take appropriate action as agreed at appraisal.
To participate in clinical audit as necessary.
To maintain a personal and professional appearance as per EPUT uniform policy.
Ensure safe practice is maintained by full awareness of key guidance in relation to annual leave, off duty and sickness reporting.
Adhere to the Trusts Lone Working Policy and Procedural guidelines.
ADDITIONAL DUTIES

To participate in the staff appraisal process and to undertake for any staff you manage
To keep yourself updated on all matters relating to Trust policy
To provide management supervision where appropriate
