Summary
Applications are invited from experienced Care Coordinators seeking to take the next step as Care Co-ordinator Manager of our clients Complex Health & Social Care and Mental Health service based in Bristol.
With high standards of clinical governance, this is a Complex Care and Mental Health organisation clinically led by both senior Mental Health and General Nurses.
Their collective experience in complex physical and mental health care means they can work with clients who have very complex needs, providing highly personalised, client-centred care in their own homes or community settings.
Rated ‘Good’ by the Care Quality Commission (England) and registered with the Care Inspectorate Wales, you will join an experienced and stable team of committed staff, supported by senior clinicians to meet our clients’ evolving needs.
Reporting directly to the Group CEO you will;
- Be responsible for leading and managing the Care Coordination Service, providing centralised oversight of all Care Coordinators across the organisation.
- Ensure that care coordination operates as a consistent, responsive and effective organisational function
- Centrally line-manage through the Pride Care Coordination Service.
- Work closely with Registered Managers, operational managers, clinical staff and senior leadership while maintaining a clear distinction between care coordination responsibilities and the statutory and regulatory responsibilities of Registered Managers.
Person requirements:
•
At least two years experience as a Care Co-ordinator within health or social care
•
Experience managing or supervising staff
•
Strong experience of operational coordination
•
Experience managing competing operational priorities
•
Experience dealing with staffing and rota pressures
•
Experience monitoring performance and holding staff accountable
•
Experience working collaboratively with multidisciplinary or multi-departmental teams
•
Understanding of safeguarding, incident management and risk escalation
•
Strong organisational and problem-solving skills
•
To have a driving licence and own vehicle (mileage expensed)
•
Right to work in the UK, not requiring sponsorship
