Summary

To provide high standards of evidence-based patient centred care to cardiac patients. To work with the team to maintain the efficient running of the ward and contribute to and deputise in the absence of more senior staff. The post holder will communicate highly complex information to patients, relatives and MDT members. This may be sensitive on occasions. A range of interpersonal skills are required to do this e.g. motivational, negotiating, persuasion, empathising, counselling, reassurance skills, and the anility to break bad news. The post holder is required to manage time effectively to maintain high standards of nursing care within the challenging environment of the Cardiology ward. To respond and care for unpredictable and diverse groups of Cardiac patients and their relatives/carers. To improve the patients experience and environment through full explanation of treatments, procedures and expectations. Assess, plan implement and evaluate individual patient care. Co-ordinate patient centred care from admission to discharge. Maintain accurate patient records. Act as a patients advocate. To promote the health and empowerment of patients. Mentor, supervisor and educator of students and other staff as appropriate. Act as a resource to the team in an area of special interest. Venepuncture and cannulation skills are desirable if these are not possessed there is an expectation that the appropriate training will be undertaken. Aid in the planning and implementing of safe discharge pathways for patients in order to maintain safety and patient wellbeing.
