Summary
Assess, plan, implement and evaluate individualised patient care plans. Administer medications safely in line with Hospice policies and professional standards. Provide symptom management, wound care and emotional support to patients and families. Maintain accurate patient records and communicate effectively with the multidisciplinary team. Supervise and support Healthcare Assistants, students and other colleagues as appropriate. Identify and report risks, complete assessments and contribute to continuous service improvement. Advocate for patients and ensure their wishes and needs are represented within the care team. Maintain compliance with infection control, confidentiality and health and safety requirements.
