Summary
Experience Significant post-registration clinical experience. Experience of working autonomously at an advanced clinical practice level. Experience of advanced clinical assessment, diagnosis and clinical decision-making. Experience of managing patients with frailty, long-term conditions and complex healthcare needs. Experience of risk assessment and management of deteriorating patients. Experience of multidisciplinary team working across health and social care settings. Experience of developing personalised care plans and supporting care coordination. Experience of managing patients within community, care home or domiciliary settings. Experience of delivering holistic care to frail, elderly and housebound patients.
