Summary

Are you a Registered Nurse with strong clinical assessment skills who wants to improve outcomes for people with complex and long-term health needs? 

We are looking for aCHC Senior Clinical Assessorto join our Continuing Healthcare (CHC) and Complex Care Team. This is an opportunity to use your clinicalexpertiseto support adults with complex needs and help ensure they receive safe,personalisedand coordinated care. 

You will undertake comprehensive assessments, coordinate and review care packages, and support individuals and their families to understand and navigate the CHC process. Working across health and social care, you will collaborate with multidisciplinary teams, care providers, localauthoritiesand partner organisations to ensure people receive the right support for their assessed needs. 

You will also contribute to safeguarding, risk management, qualityassuranceand service improvement.

As aCHC Senior Clinical Assessor,you will assess, plan,coordinateand review care for adults with complex, long-term health needs in line with the National Framework for NHS Continuing Healthcare and the statutory responsibilities of the ICB. 

Your key responsibilities will include: 

• Undertaking comprehensive clinical assessments to support CHC eligibility decisions. 

• Developing,coordinatingand reviewing personalised packages of care based on assessed needs. 

• Working collaboratively with individuals, families, multidisciplinary teams, local authorities, careprovidersand other partners. 

• Supporting the commissioning and brokerage ofappropriate careandmonitoringits quality and effectiveness. 

• Identifyingand managing clinical risk and responding appropriately to safeguarding concerns. 

• Supporting individuals and families to understand CHC decisions,processesand available routes of appeal. 

• Monitoring care providers and contributing to quality assurance. 

• Promoting safe,effectiveand person-centred care for people with complex health and social care needs. 

• Contributing to service improvement and consistent application of the CHC framework.
