Summary
Job Purpose To work within the multi-disciplinary Community Stroke Team to provide assessment and rehabilitation for patients who have had a stroke and fit the criteria for the Stroke Service. To deliver an integrated rehabilitation service by providing generic assessments of social care and health needs by working across professional boundaries and within legislation, policies and procedures and core values. Dimensions To work within the City catchment area, working with patients, their families and carers in a variety of settings including patients own home, care homes or clinic setting, setting up assessment and rehabilitation programmes for patients, carers and families To participate actively in the services training strategy, including in-service training and multi-agency training. Key Responsibilities Clinical To undertake holistic assessment of patients needs who have been referred to the service covering all aspects of peoples lives that may have been impacted by a stroke. To work alongside clinicians to interpret assessment information and plan and progress their intervention using judgement skills to decide upon and recommend best course of action. To refer patients to external agencies and specialists when their needs are beyond the assistant practitioner role under the guidance of clinicians To accept responsibility for designated caseload, working independently taking an active role in promoting independence in all aspects of a persons life which may have been impacted by a stroke including personal and domestic activities of daily living, physical abilities, cognition, mental health and social, meeting the needs of the patient. To potentially co-ordinate and lead exercise classes for stroke clients across the community. To promote wellbeing, safe mobility, transfers, and effective communication. To provide instruction, advice and intervention to patients and carers in line with the multi-disciplinary team guidelines. To assess, provide, fit, and demonstrate use of a wide range of equipment to promote independence. To assess for and commission a range of adaptations in line with the multi-disciplinary team guidelines. To assess for and commission social care services in line with the multi-disciplinary team guidelines. To attend and contribute to team meetings and case discussions using effective communication. To maintain comprehensive and accurate records in line with employers polices Managerial and Leadership. To provide support to other Assistant Practitioners, and Rehabilitation Support Workers as appropriate. Education and Development To ensure knowledge, skills and competencies are kept up to date and complete all relevant competencies within recognised time frames. To be involved in formal appraisal systems, supervision, and be active in identifying aims and objectives for personal development in line with departmental policy. Clinical Governance. To maintain a safe working environment and be aware of the necessity to comply with health and safety regulations. To have regard at all times for the confidential nature of the work by adhering to confidentiality legislation and not to disclose information to unauthorised parties. To actively promote equality and diversity within service delivery and peoples individual rights. The post holder will comply with current legislation, the organisations policies, procedures and core values.
