Summary

To support the Community Nursing Service in the provision of care for housebound clients by assisting with clinical procedures and undertaking delegated duties unsupervised. This will involve direct care with clients (within their own homes, or care homes) who have been admitted to the care of the Community Nursing Service. The post holder will be supported and supervised by the senior clinician but will work regularly without supervision. This role supports the proactive monitoring of clients in order to promote and sustain their independence and to prevent unnecessary hospital admissions.
