Summary

Key responsibilities and tasks 1. To support the Neighborhood Health Implementation Programme (NNHIP) pilot work across the PCN 2. To support the work of the PCN multi-disciplinary team (MDT) to enable them to provide high quality patient care. 3. To support the PCN Patient Care Projects and Services 4.
