To provide and receive highly complex, sensitive and contentious information, communicating unwelcome decisions to members of the public and using interpersonal skills to overcome barriers to acceptance where there may be hostility and antagonism. To review and reassess at agreed intervals the progress being made by the individual alongside the MDT, the individual and their representatives. To have consideration for the principles of the Mental Capacity Act and to work within them. To review any service and care, commissioned in regards to quality and appropriateness to the individuals need, to ensure that the provider is delivering the requirements of the contract. Sign posting to relevant teams and organizations. Responsible for oversight /assurance of commissioned equipment that may be required by the cohort. To plan with the MDT an appropriate exit from the funding stream including the completion of CHC checklist where this is appropriate. To ensure that person centred quality care and support is at the forefront. Work with system partners ensuring best practice is adhered to in respect of all commissioned services that support the needs of the complex/ABI/neurorehabilitation caseload. Support and provide recommendations for funding arrangements for individuals, working with multi-disciplinary teams and relevant panels as necessary. Be a link between key stakeholders to promote collaborative working and provide a seamless service for patients. Required to maintain constructive relationships with a broad range of internal and external stakeholders, including acute hospitals, care providers, community health providers, voluntary sector organisations and local authorities. Participate in relevant internal and external working groups/projects, services, and initiatives which may be highly complex, sensitive, political, and contain contentious information with the aim of providing information and analytical advice to the teams. Work with members of the team to develop and implement data collection systems that will provide accurate and timely data. Communicate information, risks, issues and dependencies, including briefings and reports to teams and a range of internal and external colleagues. In this role, you will work flexibly beyond your immediate team, building relationships, sharing knowledge and connecting people and priorities to support joined-up working. This includes being proactive in reaching across organisational and team boundaries, contributing to shared goals, and taking a wider system view of your work.
