Summary

The post holder will work as a Salaried GP within the practice, providing high-quality, patient-centred general medical services to our registered patient population. The successful candidate will work collaboratively with the practice team and wider multidisciplinary team to provide safe, effective and compassionate care. The role will involve the assessment, diagnosis and management of a wide range of acute and chronic conditions, together with preventative healthcare, health promotion and appropriate referral to other services. We are looking for a GP who is committed to delivering excellent patient care, works well as part of a team and is keen to contribute to the continued development of the practice. Key Responsibilities The post holder will: Provide face-to-face and remote consultations as appropriate. Assess, diagnose and manage a wide range of clinical presentations. Provide appropriate treatment and management of acute and chronic conditions. Develop management plans in partnership with patients, taking account of their individual needs and preferences. Prescribe safely and appropriately in accordance with current clinical guidance, local policies and professional standards. Review and manage patients with long-term conditions and complex healthcare needs. Provide preventative healthcare, health promotion and lifestyle advice. Undertake appropriate clinical examinations and arrange relevant investigations. Review and act appropriately on laboratory results, imaging, correspondence and other clinical information. Provide appropriate follow-up and safety-netting advice to patients. Make appropriate referrals to secondary care, community services and other healthcare professionals. Liaise effectively with hospitals, community services and other external agencies involved in patient care. Maintain accurate, contemporaneous and confidential patient records. Ensure appropriate clinical coding and documentation of consultations and clinical decisions. Participate in appropriate clinical and practice meetings. Contribute to clinical audits, significant event reviews and quality improvement initiatives. Contribute to the development and implementation of practice policies and procedures. Participate in clinical governance and patient safety activities. Recognise and appropriately respond to safeguarding concerns. Maintain confidentiality and comply with information governance requirements. Work in accordance with infection prevention and control procedures. Maintain professional registration and undertake appropriate continuing professional development. Keep up to date with relevant clinical guidance, legislation and professional standards. Contribute positively to the practice team and support effective multidisciplinary working. Clinical Care The post holder will provide comprehensive general medical care to patients of all ages within the scope of their competence. This will include the management of undifferentiated and established illness, assessment of new symptoms, management of long-term conditions and appropriate preventative care. The GP will use clinical judgement to determine appropriate investigation, treatment, monitoring, referral and follow-up. Patients should be involved in decisions about their care wherever appropriate, with clear communication regarding treatment options and safety-netting. The post holder will recognise situations requiring urgent or emergency intervention and take appropriate action. Multidisciplinary Working The post holder will work closely with GPs, nurses, healthcare professionals, administrative staff and other members of the wider practice team. The GP will contribute to effective communication and coordination of patient care and will work collaboratively with colleagues to ensure that patients receive appropriate and timely support. The post holder will respect the skills and expertise of other members of the team and contribute to a positive, supportive and professional working environment. Clinical Governance and Patient Safety The post holder will contribute to maintaining high standards of clinical governance and patient safety within the practice. Responsibilities will include: Working within relevant professional and regulatory standards. Participating in clinical audit and quality improvement. Contributing to significant event reviews and learning from incidents. Reporting and appropriately managing clinical incidents and near misses. Following practice policies and procedures. Maintaining appropriate clinical documentation. Participating in safeguarding processes. Supporting infection prevention and control. Maintaining patient confidentiality and information security. Identifying and escalating risks appropriately. Safeguarding The post holder will have a responsibility to safeguard children, young people and adults at risk. The GP will recognise and appropriately respond to safeguarding concerns and will follow relevant legislation, national guidance and practice procedures. The post holder will work collaboratively with colleagues and external agencies where necessary to ensure appropriate safeguarding arrangements are in place. Communication The post holder will communicate effectively and professionally with patients, relatives, carers, colleagues and external healthcare professionals. The GP will provide information in a clear and appropriate manner and will demonstrate sensitivity and respect when discussing potentially difficult or sensitive issues. The post holder will support shared decision-making and encourage patients to take an active role in managing their health. Confidentiality and Information Governance The post holder must maintain patient confidentiality at all times and comply with relevant legislation and practice policies relating to data protection and information governance. All patient information must be handled securely and appropriately. Clinical records should be accurate, complete, contemporaneous and maintained in accordance with practice requirements. Professional Development The post holder will maintain their professional registration and undertake continuing professional development appropriate to their role. The GP will keep their clinical knowledge and skills up to date and participate in appraisal and revalidation requirements as applicable. The practice is supportive of professional development and welcomes clinicians who wish to develop areas of clinical interest that may benefit both patients and the practice. Teamwork and Practice Development The post holder will contribute positively to the continued development of the practice. We welcome ideas and encourage staff to identify opportunities to improve patient care, clinical processes and the patient experience. The GP will participate in relevant practice meetings and discussions and contribute constructively to service development and improvement. Person Specification Essential Registered with the General Medical Council. Inclusion on the NHS England Performers List. Appropriate qualifications and training for the role.
